BACKGROUND: Hypertension is a public health concern for many countries. The World Health Organization has established a global objective to reduce the prevalence of non-communicable diseases, including hypertension, which is associated with cardiovascular disease. Remote nursing interventions can potentially lessen the burden on the healthcare system and promote a healthier population. This systematic review aims to synthesize available evidence on the effectiveness of nursing-led telehealth interventions in reducing blood pressure in hypertensive patients. METHODS: A systematic review was conducted. The search was performed from May to June 2021, in the databases: PubMed, Scopus, Cochrane Library, Web of Science, CINAHL, and ProQuest within 2010-2021 in English, Spanish and Portuguese. Randomized controlled trials and Quasi-experimental studies were considered. This systematic review followed the criteria of the Cochrane Handbook for Systematic Reviews of Interventions, with the support of the PRISMA guidelines and registered in PROSPERO. For critical analysis, the tools of the Joanna Briggs Institute were used. RESULTS: Of the 942 articles found, six controlled clinical trials and one quasi-experimental study were selected. Different nurse-led interventions (telehealth devices, remote video consultation, calls and email alerts) have demonstrated a significant decrease in blood pressure (especially systolic blood pressure) in the intervention groups. Nurse-led interventions also effect hypertension awareness, self-efficacy, and self-control. Positive effects on lowering cholesterol, consumption of fruits and vegetables, physical activity and adherence to medication were also described. CONCLUSION: Nurse-led interventions delivered remotely have a positive effect in lowering the blood pressure of patients with hypertension. Further research is required to support strategies that will deliver the best continuous, quality, and cost-effective nursing care.

Several vaccines have been developed to control the COVID-19 pandemic. CoronaVac®, an inactivated SARS-CoV-2 vaccine, has demonstrated safety and immunogenicity, preventing severe COVID-19 cases. We investigate the safety and non-inferiority of two immunization schedules of CoronaVac® in a non-inferiority trial in healthy adults. A total of 2302 healthy adults were enrolled at 8 centers in Chile and randomly assigned to two vaccination schedules, receiving two doses with either 14 or 28 days between each. The primary safety and efficacy endpoints were solicited adverse events (AEs) within 7 days of each dose, and comparing the number of cases of SARS-CoV-2 infection 14 days after the second dose between the schedules, respectively. The most frequent local AE was pain at the injection site, which was less frequent in participants aged ≥60 years. Other local AEs were reported in less than 5% of participants. The most frequent systemic AEs were headache, fatigue, and myalgia. Most AEs were mild and transient. There were no significant differences for local and systemic AEs between schedules. A total of 58 COVID-19 cases were confirmed, and all but 2 of them were mild. No differences were observed in the proportion of COVID-19 cases between schedules. CoronaVac® is safe, especially in ≥60-year-old participants. Both schedules protected against COVID-19 hospitalization.

OBJECTIVE: To analyze the association between team climate, team characteristics and satisfaction at work in teams of the Estratégia Saúde da Família com Saúde Bucal (Family Health Strategy with Oral Health) (ESF with SB). METHODS: Cross-sectional correlational study with ESF teams with SB in the municipality of São Paulo. Universe of 1,328 teams and random sample of 124 teams with 1,231 professionals. Applied questionnaire with data teams' characterization, team climate scale, and satisfaction at work. Analysis of validity, of climate and satisfaction scores through mean among professionals in each team, cluster analysis, association between variables by Pearson's correlation and Chi-square, and tested linear regression model for the two factors of satisfaction at work. RESULTS: There was a directly proportional association between team climate and satisfaction at work. The better the climate with regard to team goals, the greater the intrinsic satisfaction at work and with the physical environment. The better the climate with regard to team goals and task orientation, the greater the satisfaction with hierarchical relations. The group with best team climate reported higher percentage of teams ranked with better satisfaction at work, and in the group with the worst team climate there was higher percentage of teams with the lowest satisfaction at work. CONCLUSIONS: The study provides consistent although moderate evidence of association between favorable teamwork climate and job satisfaction in ESF with SB. It emphasizes the dimensions of climate, common goals and task orientation, and may serve as subsidy for management and permanent education of teams, aiming at the quality of care to the health needs of users, family and community in APS.

BACKGROUND: The development of effective vaccines against coronavirus disease 2019 is a global priority. CoronaVac is an inactivated severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccine with promising safety and immunogenicity profiles. This article reports safety and immunogenicity results obtained for healthy Chilean adults aged ≥18 years in a phase 3 clinical trial. METHODS: Volunteers randomly received 2 doses of CoronaVac or placebo, separated by 2 weeks. A total of 434 volunteers were enrolled, 397 aged 18-59 years and 37 aged ≥60 years. Solicited and unsolicited adverse reactions were registered from all volunteers. Blood samples were obtained from a subset of volunteers and analyzed for humoral and cellular measures of immunogenicity. RESULTS: The primary adverse reaction in the 434 volunteers was pain at the injection site, with a higher incidence in the vaccine than in the placebo arm. Adverse reactions observed were mostly mild and local. No severe adverse events were reported. The humoral evaluation was performed on 81 volunteers. Seroconversion rates for specific anti-S1-receptor binding domain (RBD) immunoglobulin G (IgG) were 82.22% and 84.44% in the 18-59 year age group and 62.69% and 70.37% in the ≥60 year age group, 2 and 4 weeks after the second dose, respectively. A significant increase in circulating neutralizing antibodies was detected 2 and 4 weeks after the second dose. The cellular evaluation was performed on 47 volunteers. We detected a significant induction of T-cell responses characterized by the secretion of interferon-γ (IFN-γ) upon stimulation with Mega Pools of peptides from SARS-CoV-2. CONCLUSIONS: Immunization with CoronaVac in a 0-14 schedule in Chilean adults aged ≥18 years is safe, induces anti-S1-RBD IgG with neutralizing capacity, activates T cells, and promotes the secretion of IFN-γ upon stimulation with SARS-CoV-2 antigens.

La pandemia Covid-19 ha impactado en el bienestar psicológico a nivel mundial y los estudiantes universitarios no están exentos, manifestando respuestas emocionales-patológicas como estrés, ansiedad y depresión provocadas por factores individuales, socio-familiares y académicos. El objetivo es conocer los tipos de estrategias de afrontamiento en 100 estudiantes de una universidad privada de Huancayo frente a la Covid-19, determinando la eficacia de un programa virtual. La investigación fue cuantitativa-descriptiva de corte trasversal y aplicada, mediante un programa virtual denominado "Cuidando mi salud psicológica" con 16 sesiones de 50 minutos/3 semanas, entre septiembre-noviembre 2021, el procesamiento de datos se realizó mediante el uso de estadística descriptiva y medidas de tendencia central. Los resultados determinaron 14 principales factores generadores de estrés y ansiedad en los estudiantes, categorizados en 3 núcleos; factor individual, socio-familiar y académico. El grupo T0 (no diagnosticado) y T1 (diagnosticado) expresaron como factor individual el temor a enfermar de Covid-19 con 0,72 y 0,88, en el factor socio-familiar se evidenció en T0 con conflictos con los integrantes de la familia (0,52) y T1 temor de contagiar personas del entorno cercano (0,62). En el factor académico fue la adaptación a otras modalidades de estudio con 0,56 (T0) y 0,42 (T1). La influencia del PSV sobre las estrategias de afrontamiento fue moderada, positiva y altamente positiva a excepción del grupo T0 en la evitación de problemas que paso de medio 7,57 (bajo) a 8,68 (medio) con influencia del PSV negativa. Demostrando así, la eficacia del programa virtual "Cuidando mi salud psicológica"(AU)


The Covid-19 pandemic has impacted psychological well-being worldwide and university students are not exempt, manifesting emotional-pathological responses such as stress, anxiety and depression caused by individual, socio-family and academic factors. The objective is to know the types of coping strategies in 100 students from a private university in Huancayo against Covid-19, determining the effectiveness of a virtual program. The research was quantitative-descriptive, cross-sectional and applied, through a virtual program called "Caring for my psychological health" with 16 sessions of 50 minutes/3 weeks, between September-November 2021, the data processing was carried out through the use of statistics descriptive and measures of central tendency. The results determined 14 main factors that generate stress and anxiety in students, categorized into 3 cores; individual, socio-family and academic factor. The group T0 (undiagnosed) and T1 (diagnosed) expressed as an individual factor the fear of getting sick from Covid-19 with 0.72 and 0.88, in the socio-family factor it was evidenced in T0 with conflicts with the members of the family (0.52) and T1 fear of infecting people in the immediate environment (0.62). In the academic factor it was the adaptation to other study modalities with 0.56 (T0) and 0.42 (T1). The influence of PSV on coping strategies was moderate, positive and highly positive, except for the T0 group in problem avoidance, which went from a mean of 7.57 (low) to 8.68 (medium) with a negative influence of PSV. Thus demonstrating the effectiveness of the virtual program "Caring for my psychological health"

PURPOSE/AIMS: The aim of this study was to explore the perceptions that experienced and highly specialized nurses have of the clinical nurse specialist (CNS) role through description of the registered nurses' (RNs') experiences. DESIGN: This study used a qualitative descriptive design. METHODS: Interviews were conducted with 6 RNs (2 managers, 1 chief nursing officer, 2 educators, and 1 clinician) and 32 RNs who participated in 5 focus groups. Participants were purposively sampled from intensive care units and emergency departments from 4 public and private hospitals. The analysis of the narratives and field notes used thematic content analysis. RESULTS: Common aspects of the CNS competencies and the experienced RN were recognized. These included direct patient care, assessment, and mentoring, with important differences in management, research, and coaching competencies. CONCLUSION: The findings could facilitate the development of a master's program for the CNS in Chile. Formally recognizing the skills and clinical experiences of expert RNs may motivate nurses to pursue a master's degree that prepares them for advanced practice.

Background: The ongoing COVID-19 pandemic has had a significant impact worldwide, with an incommensurable social and economic burden. The rapid development of safe and protective vaccines against this disease is a global priority. CoronaVac is a vaccine prototype based on inactivated SARS-CoV-2, which has shown promising safety and immunogenicity profiles in pre-clinical studies and phase 1/2 trials in China. To this day, four phase 3 clinical trials are ongoing with CoronaVac in Brazil, Indonesia, Turkey, and Chile. This article reports the safety and immunogenicity results obtained in a subgroup of participants aged 18 years and older enrolled in the phase 3 Clinical Trial held in Chile. Methods: This is a multicenter phase 3 clinical trial. Healthcare workers aged 18 years and older were randomly assigned to receive two doses of CoronaVac or placebo separated by two weeks (0-14). We report preliminary safety results obtained for a subset of 434 participants, and antibody and cell-mediated immunity results obtained in a subset of participants assigned to the immunogenicity arm. The primary and secondary aims of the study include the evaluation of safety parameters and immunogenicity against SARS-CoV-2 after immunization, respectively. This trial is registered at clinicaltrials.gov ( NCT04651790 ). Findings: The recruitment of participants occurred between November 27 th , 2020, until January 9 th , 2021. 434 participants were enrolled, 397 were 18-59 years old, and 37 were ≥60 years old. Of these, 270 were immunized with CoronaVac, and the remaining 164 participants were inoculated with the corresponding placebo. The primary adverse reaction was pain at the injection site, with a higher incidence in the vaccine arm (55.6%) than in the placebo arm (40.0%). Moreover, the incidence of pain at the injection site in the 18-59 years old group was 58.4% as compared to 32.0% in the ≥60 years old group. The seroconversion rate for specific anti-S1-RBD IgG was 47.8% for the 18-59 years old group 14 days post immunization (p.i.) and 95.6% 28 and 42 days p.i. For the ≥60 years old group, the seroconversion rate was 18.1%, 100%, and 87.5% at 14, 28, and 42 days p.i., respectively. Importantly, we observed a 95.7% seroconversion rate in neutralizing antibodies for the 18-59 years old group 28 and 42 days p.i. The ≥60 years old group exhibited seroconversion rates of 90.0% and 100% at 28 and 42 days p.i. Interestingly, we did not observe a significant seroconversion rate of anti-N-SARS-CoV-2 IgG for the 18-59 years old group. For the participants ≥60 years old, a modest rate of seroconversion at 42 days p.i. was observed (37.5%). We observed a significant induction of a T cell response characterized by the secretion of IFN-γ upon stimulation with Mega Pools of peptides derived from SARS-CoV-2 proteins. No significant differences between the two age groups were observed for cell-mediated immunity. Interpretation: Immunization with CoronaVac in a 0-14 schedule in adults of 18 years and older in the Chilean population is safe and induces specific IgG production against the S1-RBD with neutralizing capacity, as well as the activation of T cells secreting IFN-γ, upon recognition of SARS-CoV-2 antigens. RESUMO OBJETIVO Analisar associação entre clima de equipe, características das equipes e satisfação no trabalho em equipes da Estratégia Saúde da Família com Saúde Bucal (ESF com SB). MÉTODOS Correlacional transversal com equipes da ESF com SB no município de São Paulo. Universo de 1.328 equipes e amostra aleatória de 124 equipes com 1.231 profissionais. Aplicado questionário com dados de caracterização das equipes, escala de clima de equipe e de satisfação no trabalho. Realizada análise de validade, dos escores de clima e satisfação via média dos profissionais de cada equipe, análise de agrupamentos, associação entre variáveis pela correlação de Pearson e Qui-Quadrado e testado modelo de regressão linear para os dois fatores de satisfação no trabalho. RESULTADOS Verificou-se associação diretamente proporcional entre clima de equipe e satisfação no trabalho. Quanto maior clima com relação a objetivos da equipe, maior satisfação intrínseca no trabalho e com ambiente físico, quanto maior clima com relação a objetivos da equipe e orientação para as tarefas, maior satisfação com as relações hierárquicas. Verificou-se no grupo com melhor clima de equipe, maior porcentagem de equipes classificadas com melhor satisfação no trabalho e no grupo de pior clima de equipe, maior porcentagem de equipes com a mais baixa satisfação no trabalho. CONCLUSÕES O estudo traz evidências consistentes, embora moderadas, de associação entre clima favorável ao trabalho em equipe e satisfação no trabalho na ESF com SB, com destaque para as dimensões de clima, objetivos comuns e orientação para a tarefa, constituindo subsídio para a gestão e educação permanente das equipes, visando a qualidade da atenção às necessidades de saúde de usuários, família e comunidade na APS.

BACKGROUND: The burden of treatment can overwhelm people living with type 2 diabetes and lead to poor treatment fidelity and outcomes. Chronic care programs must consider and mitigate the burden of treatment while supporting patients in achieving their goals. OBJECTIVE: To explore what patients with type 2 diabetes and their health providers consider are the workload and the resources they must mobilize, i.e., their capacity, to shoulder it. METHODS: We conducted focus groups comprised of 30 patients and 32 clinicians from three community health centers in Chile implementing the Chronic Care Model to reduce cardiovascular risk in patients with type 2 diabetes. Transcripts were analyzed using thematic content analysis techniques illuminated by the Minimally Disruptive Medicine framework. FINDINGS: Gaining access to and working with their clinicians, implementing complex medication regimens, and changing lifestyles burdened patients. To deal with the distress of the diagnosis, difficulties achieving disease control, and fear of complications, patients drew capacity from their family (mostly men), social environment (mostly women), lay expertise, and spirituality. Clinicians found that administrative tasks, limited formulary, and protocol rigidity hindered their ability to modify care plans to reduce patient workload and support their capacity. CONCLUSIONS: Chronic primary care programs burden patients living with type 2 diabetes while hindering clinicians' ability to reduce treatment workloads or support patient capacity. A collaborative approach toward Minimally Disruptive Medicine may result in treatments that fit the lives and loves of patients and improve outcomes.

METHOD: Quasi-experimental study of interprofessional education intervention. Seventeen Primary Health Care teams (95 professionals) participated in the study, of which nine teams (50 professionals) composed the intervention group and eight teams (45 participants) composed the control group. The team climate inventory scale and a questionnaire on knowledge about management of chronic conditions in Primary Health Care were applied before and after intervention. Type I error was fixed as statistically significant (p<0.05). RESULTS: In the analysis of knowledge about management of chronic conditions, the teams that participated in the interprofessional education intervention presented higher mean post-intervention increase than the teams of the control group (p < 0.001). However, in the analysis of both groups, there was no significant variation in the teamwork climate scores (0.061). CONCLUSION: The short interprofessional education intervention carried out during team meetings resulted in improved apprehension of specific knowledge on chronic conditions. However, the short intervention presented no significant impacts on teamwork climate.

Objetivo: Avaliar o efeito da educação interprofissional no clima de equipes da Atenção Primária à Saúde e na apropriação de conhecimentos sobre o manejo das condições crônicas não transmissíveis. Método: estudo quase-experimental de intervenção de educação interprofissional. Participaram 17 equipes da Atenção Primária (95 profissionais), das quais nove equipes (50 profissionais) compuseram o grupo-intervenção e oito equipes (45 participantes), o grupo-controle. A escala de clima de equipe e um questionário referente aos conhecimentos sobre o manejo das condições crônicas na Atenção Primária foram aplicados antes e após a intervenção. Fixou-se erro tipo I como estatisticamente significativo (p < 0,05). Resultados: na análise dos conhecimentos sobre manejo das condições crônicas, as equipes que participaram da intervenção de educação interprofissional apresentaram aumento médio pós-intervenção maior do que as equipes do grupo-controle (p < 0,001). Contudo, na análise de ambos os grupos, não houve variação significativa nos escores de clima do trabalho em equipe (0,061). Conclusão: intervenção de educação interprofissional curta realizada durante as reuniões de equipe resultou em melhoria na apreensão dos conhecimentos específicos sobre as condições crônicas. Entretanto, a intervenção de curta duração não apresentou impactos significativos no clima do trabalho em equipe.

Método: Estudio cuasi-experimental de intervención educativa interprofesional. Participaron 17 equipos de Atención Primaria (95 profesionales); de estos, nueve equipos (50 profesionales) formaron el grupo de intervención, y ocho (45 participantes) el grupo de control. Se aplicaron antes y después de la intervención una Escala sobre el ambiente del personal y un cuestionario sobre los conocimientos acerca del manejo de condiciones crónicas. El error tipo I se consideró como estadísticamente significativo (p<0,05). Resultados: En el análisis de conocimientos acerca del manejo de condiciones crónicas, los equipos que participaron en la intervención educativa interprofesional tuvieron un aumento promedio en el posintervención superior a los equipos del grupo de control (p <0,001). Sin embargo, en el análisis de ambos grupos, no hubo variación significativa en los puntajes en el ambiente de trabajo en equipo (0,061). Conclusión: La intervención de educación interprofesional a corto plazo realizada durante las reuniones del equipo mejoró la comprensión de los conocimientos específicos sobre las condiciones crónicas. Pero no tuvo el mismo impacto significativo en el ambiente de trabajo en equipo.

INTRODUCTION: The health organizations of today are highly complex and specialized. Given this scenario, there is a need for health professionals to work collaboratively within interprofessional work teams to ensure quality and safe care. To strengthen interprofessional teamwork, it is imperative that health organizations enhance strategic human resources management by promoting team member satisfaction. OBJECTIVE: To analyze the satisfaction of members in interprofessional teams and to explore interpersonal relationships, leadership, and team climate in a hospital context. METHODOLOGY: This study is an explanatory sequential mixed methods (quantitative/qualitative) study of 53 teams (409 professionals) at a university hospital in Santiago, Chile. The first phase involved quantitative surveys with team members examining team satisfaction, transformational leadership, and team climate. Social network analysis was used to identify interactions among team members (cohesion and centrality). The second phase involved interviews with 15 professionals belonging to teams with the highest and lowest team satisfaction scores. Findings of both phases were integrated. RESULTS: Significant associations were found among variables, and the linear regression model showed that team climate (ß = 0.26) was a better predictor of team satisfaction than team leadership (ß = 0.17). Registered nurse was perceived as the profession with the highest score on the transformational leadership measure (mean = 64), followed by the physician (mean = 33). Team networks with the highest and lowest score of team satisfaction showed differences in cohesion and centrality measures. Analysis of interviews identified five themes: attributes of interprofessional work; collaboration, communication, and social interaction; interprofessional team innovation; shared leadership; and interpersonal relationship interface work/social. Integration of findings revealed that team member satisfaction requires participation and communication, common goals and commitment for patient-centered care, clear roles and objectives to support collaborative work, and the presence of a transformational leader to strengthen well-being, dialog, and innovation. CONCLUSIONS: Results have the potential to contribute to the planning and decision-making in the field of human resources, providing elements to promote the management of health teams and support team member satisfaction. In turn, this could lead to job permanence especially where the local health needs are more urgent.

The surgical process requires the coordination of a number of professionals who understand their own roles and responsibilities, as well as those of the team. In the perioperative setting, expectations are established around behaviors and competencies of every team member. These expectations are influenced by knowledge, training and experience, and may ultimately influence results and the ability to adapt and respond to work demands. In Chile, there exists an ambiguity and lack of definition in the role of the nurse. The objective of this study was to examine the healthcare team's perception of the current role of the perioperative nurse, as well as the expected and desired characteristics of the role from the team's perspective. A qualitative, descriptive case study was carried out, using semi-structured interviews conducted with a purposive sample of surgeons, anaesthesiologists, professional nurses and technical nurses from three hospitals in Santiago, Chile. The accounts were analysed using an inductive, thematic format. It was found that the current perioperative nursing role, with a predominance of administrative charting, recordkeeping and guidelines for the management of safety, quality control and human and material resources, restricts direct patient care. Expected characteristics of the role included comprehensive theoretical and practical training and the development of relational skills for teamwork, direct patient care and advocacy in the surgical context. These results provided initial steps towards redefining the role of the perioperative nurse, strengthening collaborative efforts and optimising patient care during a time of high vulnerability.

Introducción: Los padres sienten miedo y ansiedad antes de la cirugía de un hijo(a), lo que dificulta su preparación para la cirugía. Estos sentimientos podrían ser aliviados con adecuada información preoperatoria. Objetivo: Determinar las necesidades de información preoperatoria de los padres de niños que serán operados electivamente. Pacientes y método: Se encuestó a padres de pacientes pediátricos sometidos a cirugía electiva. Se registraron variables demográficas de los padres. Se evaluó la información preoperatoria que recibieron o desearían haber recibido en cuanto a contenidos, medios, oportunidad, lugar e informante. Se utilizó estadística descriptiva. Resultados: Ciento trece padres fueron encuestados, y más del 80% de ellos desea recibir información preoperatoria sobre la anestesia, la cirugía, el ayuno previo, los medicamentos y las complicaciones anestésicas, la monitorización, el manejo de la vía venosa, el manejo del dolor, la alimentación postoperatoria, el control de ansiedad, la sala de hospitalización y la de recuperación, y la entretención en recuperación. La mayoría desea ser informado verbalmente, 1 a 2 semanas antes y no el día de la cirugía; que el informante sea el cirujano en su consulta. Además, desean información a través de folletos, vídeos o talleres de simulación. Conclusiones: Los padres necesitan información preoperatoria completa de la anestesia, la cirugía y los cuidados postoperatorios, entregada verbalmente y en forma anticipada.
